How can I be sure the Ministère has received my online form?
An onscreen confirmation of receipt appears when you press the “Send” button, confirming that the Ministère has received your request.
Are there any fees for these services?
Fees are charged only for grade review requests. Such requests will be processed only upon receipt of a money order or certified cheque for $11.60, payable to the Minister of Finance.
Why is so much personal information required in order to process my request?
Your first and last names, permanent code, the institution where you were registered for the examination, and the date you took the examination are essential pieces of information for processing your request.
When combined with the above information, your date of birth and the first and last names of one of your parents permit the Ministère to confirm your identity and to avoid responding to a request about you without your authorization, in accordance with the Act respecting Access to documents held by public bodies and the Protection of personal information.
Your mailing address tells us where to send the response to your request, and your email address and telephone number allow us to communicate with you quickly should we require additional information.
How many days will it take to receive a response?
The Ministère undertakes to process requests for duplicate statements of marks within five business days of receipt, requests for copies of examination papers within 10 business days of receipt, and requests for grade reviews within 20 business days of receipt of both the request and the money order or certified cheque.
Please allow for mail delivery time. Canada Post’s website indicates a delivery time of approximately three days for in-province delivery.
You can therefore expect to receive a response to your request within 8 to 23 days, depending on the nature of your request.
Do all the fields on the form have to be filled out for my request to be processed?
All mandatory information required on the form must be provided when making a request for a document or service, as such information is essential in order for the request to be processed.
Is there another way to submit my request other than by using the online form?

Can you send me the document I’ve requested by any means other than by mail?
No. Regular mail is the most secure option to protect the personal information contained in this type of document.
Can the Ministère send my statement of marks or my exam paper to a third person or any other organization?
The Ministère sends the document to the mailing address of the person who made the request, i.e. the person who wrote the examination.
What is a permanent code?
Your permanent code is an alphanumeric code that is specific to you and identifies you. The Ministère assigns a code to every individual who attends school in Québec.
The format of the permanent code is LLLLCCCCCCXX, where:
- LLLL are letters corresponding to your last name followed by your first name
- CCCCCC are numbers corresponding to your date of birth
- XX are numbers generated by the Ministère
As the last two numbers cannot be logically deduced, they can only be obtained from a document containing your permanent code (e.g. transcript, diploma, report card).
May I consult or correct personal information concerning me that is held by the Ministère?
Yes. All individuals have the right to access information concerning them that is held by the Ministère and to correct any mistakes, and have the right to do so for as long as such information remains in the Ministère’s possession.
Who will have access to the personal information I provide and how will this information be used?
Only authorized personnel with the Ministère will have access to the personal information you provide, and then only insofar as is necessary to respond to your request.

Do I need to sign my request if I’m using the online form?
No. Your signature is not necessary for online requests as the personal information you are required to provide permits the Ministère to properly identify you to ensure that no request is processed without your authorization and that neither your personal information nor the response to your request is released to a third party.
However, all requests sent to the Ministère by email, mail or fax must be signed.
