Instructions – Request for a Certification of Diploma Awarded or a Duplicate Diploma of College Studies

To obtain a certification of diploma awarded or a duplicate of your Diploma of College Studies, complete the online form .

Only the permanent code, email address and comments sections are optional. If you have entered your email address, you will receive a message confirming that the Ministère has received your request.

When applying for admission to a university located in Québec, you do not need to provide a certification of diploma awarded or a duplicate of your Diploma of College Studies (DCS). With your authorization, the university can access a ministerial database that confirms the DCS(s) you have obtained.

• The Ministère does not issue college transcripts or Attestations of College Studies (ACS). To obtain a copy, please contact your college.
• To obtain a copy of a Diploma of Vocational Studies (DVS), a Secondary School Diploma (SSD) or a secondary-level achievement record, you must submit the form Request for Achievement Records and Secondary School Diplomas .
• If you wish to provide proof to an employer or any other person of the awarding of your diploma, please request a certification of diploma received (not a duplicate).
