Submitting a Document or Service Request Form

Requests for copies of examination papers or duplicate statements of marks may be submitted electronically using the online form, or by email, regular mail or fax using the paper form. 

Requests for grade reviews may be sent electronically using the online form, or by email, regular mail or fax using the paper form. However, such requests will only be processed if a money order or certified cheque for $11.60, payable to the Minister of Finance, is received by the Ministère at the following address within 10 business days of receipt of the request:

To obtain a copy of your English Test paper (1992 to 1997), you must print and fill out the form and send it by email, regular mail or fax.

1. Online forms

Fill out all mandatory fields in the online form and submit it to the Ministère. It is important to keep a copy of the onscreen confirmation message. 

If you are requesting a grade review, you must send a money order or certified cheque for $11.60, payable to the Minister of Finance, to the address provided in Section 3.

2. Forms sent by email

Print the form, fill out all mandatory fields, scan it and send it to the following email address: eule-public[a]education.gouv.qc.ca .

3. Forms sent by regular mail

Print the form, fill out the mandatory fields and mail it to the following address:

You must enclose a money order or certified cheque for $11.60 if you are requesting a grade review.

4. Forms sent by fax

Print the form, fill out the mandatory fields and send it to 418-643-1926. Please keep a copy of the transmission confirmation report.

Protection of personal information

The information provided on the Document or Service Request form enables the Ministère to confirm the identity of the person making the request and to provide proper follow-up. This information is kept for the duration of the processing of the request.

Only authorized personnel with the Ministère have access to the information provided by the person making the request. However, despite the precautions taken by the Ministère, there is always a certain risk involved in sending an email as there may be a breach of confidentiality during transmission.

It should be pointed out that, under the Act respecting Access to documents held by public bodies and the Protection of personal information, you have the right to access and correct any personal information concerning you on file with the Ministère.
